By signing and submitting this form, you acknowledge that you have read, understand, and agree to the terms outlined in our service contract below.

Notice to Consumer, you understand that Legal Document Assistants are not attorneys. CD&D Associates is not a law firm. Legal Document Assistants cannot represent you in court. Legal Document Assistants cannot advise you about your legal rights or the law.

This is a contract for the self-help services described in Part 1 below.

IMPORTANT NOTICES: Our Legal Document Assistant is registered in Santa Barbara County, Registration Number 64. To confirm registration, you may contact the Santa Barbara County clerk's office at: 805 568-2250.

Part 1, You should read and understand this entire contract before you sign it. You should understand the kinds of services that we can and cannot provide for you. We are not an attorney. We cannot perform the legal services that an attorney performs. We cannot engage in the practice of law. We cannot keep your original documents if you request that we return them to you. We cannot keep your original documents if you and I do not sign this contract or if this contract terminates (ends) for any reason. We cannot keep your original documents after all the contract services have been provided. It is a violation of California law if we keep your original documents under any of these circumstances. As required by law, CD&D Associates Legal Document Assistants have filed a bond or made a cash deposit and have registered as a Legal Document Assistant in each county where services will be performed on your behalf.

SELF-HELP SERVICES, Kinds of services that CD&D Associates legal document assistant can perform for you: Self-help services for you in connection with a legal matter in which you are representing yourself: We can type or otherwise complete legal documents that you have selected. We can provide you with general published factual legal information that has been written or approved by an attorney to help you represent yourself. We can provide you published legal documents. We can file and serve legal forms and documents as you specifically direct. If you need to be represented in court or need legal advice, then you require the services of an attorney.

I understand that work will not be started, continued, or completed until all fees have been paid to CD&D Associates and this original contract is signed and returned. I also agree that if any additional fees or costs are incurred that work will cease until these fees, and or cost are paid in full.

Please Note: Fees below do not include mandatory superior court filing fees. You may qualify for a fee waiver. If your case becomes contested or the court requires both parties to go to a settlement conference and / or mediation, there will be additional costs, as indicated below. You may need to hire any attorney to resolve your case, if your case becomes contested.

Part 2, PLEASE SELECT THE DOCUMENTS YOU WANT CD&D ASSOCIATES LEGAL DOCUMENT ASSISTANT TO PREPARE:

For contested matters, Set Settlement Conference Hearings and Set Child Custody Mediation Hearings to be determined by the agency. COURT FILING FEE (most cases require 2 filings) - $99

PROCESS SERVING – Local Service of Process: $125.00 Includes up to 3 attempts. Out of area service may require an additional charge.

You pay only for the services listed in PART 2 Fees and Cost and no others.

CANCELLATION - You may cancel this contract for any reason within 24 hours after we both have signed it. If you cancel the contract, I must immediately refund any fees which you have paid. The only fees that we may keep are fees for services which we have actually, necessarily and reasonably performed on your behalf during the 24-hour period. We cannot keep any fees for services performed during the 24-hour period unless you knew that we would perform those services and you expressly agreed in this contract that we would perform them. To cancel this contract, send us a written notice stating that you are canceling the contract. E-Mail the notice or mail it by first-class mail with the correct postage. Cancellation takes effect on the date of the postmark on the notice.
